Overview
Bier spots are small, irregularly shaped, white or hypopigmented macules that appear on the arms and legs when the extremity is in a dependent (hanging down) position. They are caused by physiological venous stasis and vasoconstriction and disappear completely when the limb is elevated or when pressure is applied. Bier spots are entirely benign and represent a normal vascular phenomenon, not a pigmentary disorder.
Key Symptoms
- Small, irregular, white or pale macules on the arms or legs
- Appear when the extremity is hanging down or in a dependent position
- Disappear completely with limb elevation or application of pressure
- No associated itching, pain, or other symptoms
- Surrounding skin may appear mildly erythematous (red), making the spots more noticeable
- Most commonly noticed in young adults

Care Instructions
- No treatment is needed as Bier spots are a normal physiological phenomenon.
- Reassurance that the condition is completely benign and harmless.

- See a dermatologist if there is any uncertainty about the diagnosis.
- Avoid unnecessary treatments or biopsies.
